kingwl 2eed448acf feat(bash): the sandboxed executor — per-call policy carrier, denial facts, runner-failure classification
dsh-bash grows the per-call policy carrier: BashExecRequest.sandboxMode
(request-optional, spec required-but-nullable — the owner pattern; resolve()
is the one explicit defaulting step) and the BashExecutor.sandboxMode
capability fact (undefined in the base class — composition truth the tool
layer can read). dsh-bash-local carries the field verbatim and confines
nothing.

dsh-bash-sandbox extends LocalBashExecutor and hands ctx.sandbox the exact
argv it is about to spawn. A denial is a RESULT FACT (the command RAN;
result.sandbox.denied is orthogonal to exitCode/signal), classified
conservatively against the wrap own dialect; a RUNNER failure outranks
denial — foreground re-throws the structured SANDBOX_UNAVAILABLE, a settled
background task stamps sandbox.runnerFailed — so a broken sandbox never
reads as a failing command and the command never runs unconfined.
dsh-tool-bash renders the markers and teaches the model not to retry around
a policy denial; escalation and per-session switching are staged follow-ups.

Adds the missing core-data-structures coverage the catalog policy
requires for non-spine seam vocabulary: the code-runtime.md sub-page
with drift-checked type-equiv blocks for all six seam types, the core.md
sub-page row, the type-equiv manifest entries, and LINK_MAP entries so
the generated service signature links CodeRunRequest/CodeRunResult;
cordis/config catalogs regenerated.

Tianyi Cui 23fb1febcd chore: keep the type-equiv manifest in its one-line-per-entry format
The previous commit rewrote the whole file through a JSON pretty-printer,
reformatting every existing entry; restore the established compact style with
the four new entries appended to the tools.md group.

Tianyi Cui d1b52a063b fix review findings: own-property and plain-JSON discipline in the schema subset
Three Codex findings on json-schema.ts, one discipline:

- required-declared and every value check now use Object.hasOwn — 'in' let
  inherited names (toString) satisfy required, dodge additionalProperties:
  false, and validate a declared property against the value's prototype
  member instead of a carried one
- isObjectLike now means PLAIN JSON object (proto chain of at most one link,
  realm-agnostic): a Date annotation or a Map-as-properties no longer passes
  structurally and serializes lossily — they fail loud as subset violations
- startInProcessRun asserts BEFORE the defensive structuredClone, so a
  hostile schema fails as OutputSchemaError, never a raw DataCloneError

Also the type-equiv catalog gap: tools.md gains the structured-output subset
vocabulary (4 blocks) with matching manifest entries. The driver index also
drops the runtime internals from its public re-export (runs acquire it
internally; no external consumer remains — see the following commit).

  carries the workflow tool; authored error-finish/cancel headers patched to
  the merged tool list and system text

Tianyi Cui c0808d5126 docs: the governing principle — every LLM request is reconstructable from the session log
The reconstructability RFC is the principle's home: model-visible ⟺
logged in both forms, the mechanism (boundary derivation + header
fold), the enforcement (write-time round-trip guard, the dev
invariant), the corollaries ranked (prefix-cache stability first), the
MiniCode lineage with the provenance arrow inverted, and the
alternatives it beat — including the stateful transmission client
whose three-design archaeology lives in PR #162.

Placements per the one-home-per-fact taxonomy: a standing-order line in
root AGENTS.md (with displacement trims to stay inside the 1,575-word
ceiling), the principle statement in architecture.md § Session Log and
its Turn Flow lines (condensed to the ratcheted 1,630 ceiling), the
request-envelope section in core-data-structures/core.md with the
LlmCallConfig paste, both review-requested FIXMEs
(FIXME(call-config-shape) beside the type, FIXME(catalog-verbs) at the
catalog's drift-gate note), cookbook rows redirected off agent/request
(tool filtering → system-prompt/assemble, plan-mode prompt → sections/
inject()), and the llm/stream JSDoc stating the frozen-request
contract. RFC index and all generated catalogs regenerated.

Tianyi Cui a7b569850b session: the request header becomes logged state — request/header events + fold/diff/apply
Every conversation request's non-content half (system prompt, tool
schemas, call config — the EpochHeader) is now recorded in the session
log: a 'request/header' full snapshot (reason 'initial' | 'resume' |
'fallback') anchors the fold at conversation birth and process
boundaries, and 'request/header-delta' events (system line-trim,
name-keyed tools delta, whole config) encode mid-run changes. The pure
trio — foldRequestHeader / diffHeader / applyHeaderDelta — reconstructs
the header any request was built under from the log alone; the writer
contract round-trip-verifies every delta with a 'fallback' snapshot
when the encoding cannot express a change (pure tool reordering), so a
well-formed log always folds cleanly. Canonical absence: empty system
and empty tools normalize to absent fields, matching request builds.

Persistence and cordis catalogs regenerated; SessionEventMap paste and
EpochHeader added to the core-data-structures session page.

Tianyi Cui 1d43ea3cd5 workflow: dynamic workflows — script-driven multi-agent orchestration
A new capability family at packages/workflow/ in the bash seam shape,
modeled on Claude Code's dynamic workflows: the model writes a JavaScript
orchestration script (export const meta = {...} + plain-JS body), a runtime
executes it, and the script — not the conversation — holds the loop, the
branching, and the intermediate results.

- dsh-workflow (ctx.workflows): abstract WorkflowService + run vocabulary
  (WorkflowRun whose result NEVER rejects) + observe-only workflow/* events
  carrying data snapshots (id + meta, never the live run), per-listener
  contained like subagent/*.
- dsh-workflow-vm: in-process node:vm engine. Meta extraction via a
  string/comment-aware scanner (template interpolation rejected; literal
  evaluated alone in an empty timed context; statement blanked line-
  preservingly so stacks keep script line numbers). Hooks: agent(prompt,
  {label, phase, schema, model}) over ctx.subagents, parallel(), pipeline()
  (no cross-stage barrier), phase(), log(), args. Fatal-vs-null discipline:
  hook misuse (unknown/deferred options, bad arguments, unsupported
  schemas, tripped caps, seam start failures, cancellation) throws fatal
  WorkflowErrors the combinators RE-THROW — never dissolved into the
  per-item null reserved for child failures. Realm boundary: inbound values
  materialized by descriptor walks that never invoke accessors (defineProperty
  copies, __proto__-safe); outbound values rebuilt in-realm via the
  context's own JSON.parse. Determinism bans (Date.now/Math.random/argless
  new Date) kept so future resume support cannot break scripts. Caps and
  timeouts are validated Config. Every hook promise carries a no-op
  rejection consumer (app-boot exits on unhandled rejections).
- dsh-tool-workflow: the model-facing workflow tool, synchronous like
  dsh-tool-subagent (start → await → try/finally dispose; abort bridged;
  non-completed → isError). Generic render card titled by a textual
  meta.name sniff. The tool description carries the authoring contract.

Wired into examples/{coding-agent,acp-agent} with explicit-ask-only
guidance. Coverage at every tier: unit (meta scanner, materializer incl.
counting-getter and __proto__ regressions, combinator semantics,
concurrency ceiling, caps, cancellation, no-unhandled-rejection abandon),
integration over the real spawn stack, with-key e2e (real two-phase run +
the tool through the registry pipeline), and a recorded ACP snapshot
scenario (workflow-run, 1 child session). RFC:
docs/rfc/implemented/feature/2026-07-05-dynamic-workflows.md (deferred
work explicitly listed). AGENTS.md budget 1575 → 1590 for the new group's
layout line.

Tianyi Cui 0ebb86e70f Implement mandatory app-attribution headers per the RFC
dsh-llm owns the vocabulary (attribution.ts): AppIdentity with the version
read from the package manifest, userAgent(), and attributionHeaders(target,
identity) over a closed AttributionTarget union ('generic' | 'openrouter').
Both adapters send the headers on every provider request — llm-deepseek in
its fetch headers, llm-pi-ai through pi-ai's StreamOptions.headers — behind
an explicit attributionTarget config (never inferred from baseURL), with
mock-server tests asserting exact wire arrival and the absence of the
OpenRouter set by default.

The RFC moves to implemented/ amended with the settled identity (the
deepseek-harness token, the DeepSeek Harness title, the planned
deepseek-ai/deepseek-harness-sdk URL behind a FIXME until that repo exists)
and the explicit-config OpenRouter decision.

Tianyi Cui de79553d4f refactor(web): drop the unconsumed observation surface
WebService exposed an observation surface nothing in production observes:
the web/providers-change event (declared, emitted on every provider
registration/disposal, rollback-yield ordered before the emit solely so a
throwing change listener unwinds the registration) and the aggregated
searchStatus()/fetchStatus() query with its WebCapabilityStatus union.
dsh-tool-web executes through ctx.web.search()/fetch() and routes on the
structured WebError codes selection throws at execution time; tool
registration follows product enablement, not provider availability. The
only listeners/callers were the web packages' own tests, and the
tool-web README / architecture.md prose claiming the tool 'reads only the
aggregated searchStatus()/fetchStatus()' had drifted from the call sites.

Remove the event declaration, both emits, and the rollback-before-emit
machinery (the plain ctx.effect disposer keeps HMR cleanup, matching
LlmService.registerAdapter). Remove searchStatus()/fetchStatus(),
resolveStatus(), and WebCapabilityStatus; the provider-private status()
stays as the execution-time selection input. Delete the listener-throw
rollback test, and rewrite every event/status assertion across the web
packages' tests onto caller-observable behavior: a successful
search()/fetch() or the structured WEB_PROVIDER_* codes. Regenerate the
cordis catalog; update the web/tool-web READMEs, the architecture.md web
paragraph, core-data-structures/web.md, and the type-equiv manifest; amend
the web capability seam RFC's facts to the shipped surface. This follows
the llm/adapter-change precedent: a boot-time backend-registry signal and
an availability probe distinct from executing both sit on the cut side of
its keep/cut criterion.

Dudu-0223 7441307251 fix(web): open WebError.code to string, aligning with other seams
The closed WebErrorCode union leaked fetch-transport details (redirect,
too-large, content-type) into the seam's shared vocabulary and made web
the only seam with a closed error-code union. Drop it and let WebError
carry an open code: string like LlmError/SubagentError; document the
codes grouped by owner (seam-neutral vs dsh-web-fetch-local transport).

Addresses tianyicui's leaky-abstraction review comment on WebErrorCode.

Tianyi Cui 30c1863755 fix(fs): address review — rename to dsh-fs-policy, fs/*-intent events, RFC currency, ENOTDIR
Rename per review naming decisions:
- package dsh-file-context → dsh-fs-policy (dir, package name, plugin name,
  tsconfig refs, importers, type-equiv manifest, generated catalog + module-graph)
- events fs/write-expectation → fs/write-intent, fs/edit-expectation → fs/edit-intent
  (fs/observed unchanged); type FsWriteExpectation → FsWriteIntent, "expectation"
  wording → "intent" throughout
- exported FileContextExec → FsPolicyExec

Make the implemented RFCs describe what shipped, not the superseded designs:
the 2026-06-17 capability-seam + tool-schemas RFCs no longer place policy on
ctx.fs or use full/partial-view authorization, and the fsspec RFC's ctx.fileContext
service prose is rewritten to the fs/* event-gate reality (freshness-based auth).
Sharpen docs/rfc/implemented/AGENTS.md: a rename is a fact to fix IN PLACE — the
"new RFC" escape hatch is for macro decision reversals only, not renames.

Code fixes from review:
- fsio.ts resolveLocalTarget/probe translate ENOTDIR (a parent path segment is a
  file) into the structured FsError taxonomy instead of leaking a raw Node error;
  resolve reports FS_NOT_FOUND, probe reports absent. Regression tests proven to
  fail on the unfixed code.
- tool-fs HMR test now asserts prompt sections (not just tool schemas) are
  withdrawn on disposal.
- fs/observed is a plain (unguarded) ctx.emit: correct the fs-policy comment,
  filesystem.md, and tool-fs module doc that wrongly claimed the tool "contains"
  a throwing listener; a throw surfaces as the tool's isError result.
- drop the false "loaded by the default product config" claim (no config wires
  the fs tools yet), the duplicate ctx.bash service-map row, the stale
  FileReadRequest catalog link-map entry, and the fs/fs README EOF blank line;
  correct the dsh-fs package.json description.

Tianyi Cui dc95a7881d feat(events): interception seams — the typed-Decision surface for hooks
Reshape the agent's interception surface so every seam returns a small, typed
Decision union, and the set covers the hook points a CC/Codex bridge (and a
native plugin) needs. "Native hooks" are not a package — a native hook is just a
cordis plugin on these canonical events; the bridges (a later PR) only translate
an external protocol onto the same surface.

dsh-agent:
- NEW agent/session-start(agent, source) emit (once before turn 1; SessionStartSource
  startup|resume|clear|compact) — a pure notification, seeds context via inject().
- NEW agent/prompt-submit waterfall → PromptDecision (allow, optionally rewriting the
  prompt or attaching additionalContext, or block).
- RESHAPE agent/turn-continuation boolean → ContinuationDecision ({action:'stop'} |
  {action:'continue', reason?}; a continue reason is recorded as next-step steering).
- New HookContext envelope (required source — inject() would mislabel a missing one).

dsh-tools: split the single tools/execute waterfall into tools/pre-execute
(PreToolDecision allow/deny/ask gate) and tools/post-execute (PostToolDecision
accept/block, optionally replacing content or attaching additionalContext). Core
dispatch sits between as plain code; the tool body keeps its inner try/catch so a
thrown tool still reaches post-execute as an isError. ToolExecutionResult gains
additionalContext (ferried to the loop's per-step buffer). Input rewrite is
deliberately NOT offered (a proposed RFC designs it consistently).

dsh-session: new `rejected` TurnEndReason — a turn whose whole prompt batch was
blocked by prompt-submit.

agent-loop firing points: session-start emitted at create (source threaded —
startup for create/fork, resume for resume()); prompt-submit per drained message
with the always-open-turn rule (a fully-blocked batch is a zero-step rejected
turn); the continuation reshape; post-tool additionalContext buffered and appended
after all tool/results (adjacency). ACP codec maps rejected→cancelled.

A worked native-plugin example (interception.spec.ts) proves all four seams compose
end-to-end through the real loop with NO hook/* events (those belong to the bridge
lib). All existing tools/execute + turn-continuation tests migrated. The
tool-subagent abort test now aborts after a microtask so it still exercises the
live onAbort bridge (execute() awaits pre-execute before the body runs).

Dudu-0223 f8e99b8740 refactor(tool-fs): consolidate read rendering; drop the fs/observed try-catch
Two cohesion cleanups on the filesystem tool package:

- Fold window.ts + types.ts + formatReadOutput into one cordis-free
  read-render.ts. Line windowing, the FileReadOutcome shape, and output
  formatting are one concern (the read tool's rendering); splitting them across
  three files added no value. read.ts is now just the tool (schema + I/O).

- Drop observe.ts and emit fs/observed with a plain ctx.emit in read/write/edit.
  The event is contractually a synchronous, side-effect-only recorder
  (file-context's listener is a WeakMap.set), so the per-call try/catch guarded
  against a contract violation that cannot happen under the shipped listener —
  defensive code for an impossible case. The event contract (dsh-fs JSDoc,
  README, RFC) is updated to state the fire-and-forget semantics plainly.

Tianyi Cui 22a89847ac feat(session): add TodoItem + todo/write event vocabulary
Add the TodoItem type and a todo/write SessionEventMap variant carrying the
whole todo list as a snapshot (last-write-wins on replay). It is NOT a
SurfaceEventType: it produces no LLM message and never reaches
deriveMessages(), so it carries no surfaceOp and stays off the surface — it is
durable, replayable UI state that rides the existing session/event emit.

Tests cover the snapshot-clone-on-append contract, last-write-wins, the
not-on-surface guarantee, and a seeded replay round-trip. Docs: session.md
gains the TodoItem type-equiv block + the event member; core.md's variant count
goes to twelve; the type-equiv manifest gains TodoItem.

Dudu-0223 90dceea0e4 refactor(fs): make dsh-file-context an event-gate plugin, not a method service
Invert the tool↔policy control flow per the file-context event-gate RFC.
dsh-tool-fs becomes the executor — it reads/writes/edits through ctx.fs
directly, owns read windowing, and dispatches fs/write-expectation /
fs/edit-expectation (single-slot waterfalls) plus a contained fs/observed
emit. dsh-file-context drops its ctx.fileContext service and becomes a pure
event-gate plugin (observed-state + read-before-edit + version-guarded
write/edit, decided on those events). The provider's version guard becomes
optional so ctx.fs alone is a complete unconstrained text-storage seam:
removing the policy plugin gracefully loses the policy instead of breaking
the tool at a service-injection boundary.

Dudu-0223 567519184b fix: address codex review round 1
- Re-validate redirect targets through validateFetchUrl before following, so a
  same-origin Location carrying credentials (or a non-http(s)/over-long URL)
  cannot bypass the transport hygiene a direct request enforces.
- Treat only DROPPED bytes as truncation: a body exactly at maxResponseBytes is
  no longer falsely flagged truncated (which emitted a spurious footer).
- Honor the declared response charset: parse the Content-Type charset and decode
  with it (rejecting unsupported labels as WEB_UNSUPPORTED_CONTENT_TYPE) instead
  of always assuming UTF-8 and returning replacement characters.
- Catalog the web seam vocabulary in docs/core-data-structures/web.md with
  type-equiv blocks + manifest entries, per the core-data-structures rule.

Dudu-0223 ef37ce3b9d refactor(fs): split filesystem seam into provider ctx.fs + policy ctx.fileContext
Implements the split-the-filesystem-seam RFC. ctx.fs shrinks to a text-storage
provider seam (resolve/stat/readText/streamText/writeText/editText with branded
FsTargetKey/FsVersion and an explicit FsWriteExpectation); the new
dsh-file-context package owns the model-facing policy (read windowing,
observed-state, write/edit freshness) as the concrete ctx.fileContext service.

Authorization is now freshness-based rather than full/partial view: a windowed
read records the file version and authorizes a later edit when the file is
unchanged, removing the dead-end where reading lines 100-150 of a large file
could not edit line 120. editText stays a provider primitive so version guard +
literal match + atomic rewrite remain one critical section, and the stale check
runs before matching so a stale edit reports FS_STALE_VERSION. tool-fs injects
fileContext, never reaching around to ctx.fs (the no-bypass contract).

Hypatia May 9cc8dc371e Merge remote-tracking branch 'origin/master' into session-surface
Reconciles the session-surface work (surfaceOp/sourceEventSeqs provenance as
the sole derivation path) with master's worktree-subagent series (fork-seed
boundary + out-of-process subagent backends).

Semantic reconciliations beyond the textual auto-merge:
- SQLite SCHEMA_VERSION: both sides bumped 2->3. Merged to a single v3 carrying
  BOTH column families — master's seed_length on `sessions` and surface's
  source_event_seqs/surface_op on `events`. writeRow + both INSERT sites bind
  the full set; the schema doc lists all three added columns as the v2->v3 gap.
- agent-loop runStep request: master's `sessionId: session.id` and surface's
  per-append surfaceOp/sourceEventSeqs coexist (different regions).
- Fork seed + surface: a fork seeds the child from the parent's LIVE events,
  which now carry surfaceOp, so the child's surface rebuilds correctly. Verified
  end-to-end — the subagent-fork replay recalls the inherited "SAFFRON" codeword
  through the seeded prefix.
- Subagent snapshot fixtures (recorded pre-surface) re-enriched via KEYLESS
  deterministic replay: only surfaceOp/sourceEventSeqs added onto existing
  recorded lines (matched by seq), no recorded value changed. Not re-recorded
  against the live API.

Gates: typecheck, test (1112), test:snapshot (14), doc-sync, lint, build,
hygiene all green.
