docs: purge chain-of-thought leakage from prose

Delete design-session citations (decision/audit/plan ordinals, stack
positions), change narration, review choreography, and reviewer-addressed
justification from comments, JSDoc, docs, READMEs, Agent Notes, tests, and
generator templates; restate every affected fact as current-state contract
prose. Fix generated docs at their sources and regenerate the catalogs and
cordis-surface regions; re-paste type-equiv blocks; update every bilingual
counterpart and re-record the pairs. Record the citation rule in the
committed-artifact-citations Agent Note.

fix(trajectory): show exact millisecond durations consistently

Drop the details-panel Duration toggle: Duration rows always show
integer milliseconds, matching the cell time column. Timeline labels
(Total/TTFT/Decoding) and step-group descriptions previously fell back
to second labels at or above one second; they now also show exact
milliseconds via the shared formatDurationMillis formatter.

fix(trajectory): display sub-second durations as exact milliseconds
...
formatElapsedSeconds rounded every duration to one decimal, so calls
under 50 ms rendered as 0 s. Durations below one second now show integer
milliseconds (29 ms); at or above one second the tenth-of-a-second label
is unchanged (1.5 s). Step group descriptions reuse the same formatter.
The details-panel Duration rows toggle between the readable label and
exact milliseconds on click, mirroring the StartedAt toggle; the shared
text-selection guard is extracted for both. tabular-nums is dropped from
detail values and the toggle buttons because SF's tnum feature widens
the decimal point and leaves excessive space after it.

Summary scroll regions keep their scrollbar thumbs transparent until the
region is hovered or contains keyboard focus, without changing the
reserved scroll geometry. Updates the package README pair and the
inspection-ledger Agent Note together.

refactor(agent-loop): drop the steering/message session event
...
Steer, inject, and followup now land as durable user/message events on the
session surface; the steering/message event type and its ConversationNode
kind are removed from the client projection. Update tests, docs, generated
catalogs, and agent notes to match, and align the steering e2e fixture and
prompt inventory assertions with the durable user/message landing.

feat(ui-trajectory): float the composer over the ledger like chat
...
The trajectory host kept the composer as a fixed flex sibling, so the
ledger never reached the viewport bottom. Anchor the composer seat
absolutely over the ledger (reusing chat's fade treatment) and have the
internal scroll panes reserve the composer's live height plus a 16px gap
so end rows and detail bodies scroll clear of the overlay.

feat(web): unify tool-row expand interaction with IN/OUT card and trajectory Inspect
...
Every expandable tool row shares one interaction (whole-row toggle,
icon-to-chevron hover preview) and one expanded body: an IN/OUT
gutter-labeled card with per-section 150px scroll caps and sticky labels.
toolRowModel derives result output and the error first line, terminalFailed
surfaces a failing exit as the collapsed row's red dot, a hover Inspect
pill jumps to the call's trajectory record through a one-shot store
handoff, and the chat view keeps its scroll offset across view switches.
