_Kerman 2a51ef85fb fix: close three seams the message-machine refactor left open
agent-loop lifecycle: dispose drains machine.done to true quiescence.
cancel()'s own running-to-idle transition can legitimately re-enter
through an automation listener (goal-session's idle drive runs
synchronously to its first await) and replace done with a fresh
admission after the single capture; teardown now re-cancels and
re-awaits until the slot stabilizes, so the scope never unwinds under a
live run.

tools: a nested concludeTurn() stages on its own execution and promotes
to the enclosing composite only on the call's authoritative successful
verdict. A post-execute policy that converts the nested success into an
error no longer lets a recovering composite stop the turn on a failed
terminal operation (the Code Mode structured-output shape).

goal-session: the driver owns its round durability barrier again. The
loop's persistence is eager write-behind with no turn-end flush, so the
old post-turn agent/error signal for flush failures never fires; a
settled round now sets needsCheckpoint and re-enters drive, flushing
before the next reservation and disarming on failure instead of queueing
an autonomous round on state that was never persisted.

_Kerman 338da9f2e0 refactor(agent): fold agentInterruptReasonOf into loop-private slot invariants
The public classifier existed to defend an exported reader against
arbitrary signals, but its only production caller is the loop reading
its own machine-private turn signal, where cancel() is the sole aborter
and always writes one frozen canonical cause. Delete the export and its
15-line structural validation: settle() states the slot invariant with
one cast, the boolean call sites ask signal.aborted directly, and the
retry veto drops entirely because a requested window already implies a
live signal (cancel() retires the window before aborting).

The abort(reason) channel and first-wins semantics are unchanged; only
the reader's publicness is gone, and with it the paranoia it required.

Tianyi Cui f8342b0a8e test(web): cover the settings surface and workspace management
Two new keyless scenarios for the functionality master gained since this
lane's base (#644 websettings, #643 workspace browser rework), both zero
model calls:

- settings-chrome: the modal shell (sidebar-foot trigger aria states,
  role=dialog, aria-current section switch to the deliberately empty
  Models, Escape + close-button paths, dialog aria golden); the Appearance
  row as the REAL theme gesture — retiring lifecycle-chrome's
  TODO(web-theme-gesture): clicking 深色 runs aria-pressed -> persisted
  dsh.theme -> body[data-ds-dark-theme] -> alias-token flip, survives
  reload, and 'system' follows the emulated OS scheme both ways; the
  Language row switches the settings-scoped copy to English (dsh.locale
  persisted, survives reload) and restores zh. Intentional reloads tear
  the SSE stream, so the spec drains exactly its own reconnect warnings —
  the tripwire still fails on unexpected connection loss.
- workspace-management: create-by-name twice through the region-header
  dialog (host-durable via ctx.workspace.list()); rename end to end —
  hover-revealed row menu (the button is display:none until the row
  hovers), duplicate-name pre-check (inline role=alert + disabled primary
  before any wire call), then workspace.rename through the real RPC,
  row update, host durability, reload survival; the flat 'In one list'
  view (section label flips, group headers drop, dsh.workspace.view
  persists across reload, grouped restored); the session hover card
  (dwell to open, closes on pointer leave). The one session row reuses
  seeded-history's committed seed — no new recording. Deliberately not
  driven: the inert menu rows and drag reorder (deferred in the note
  with re-entry triggers).

Agent Note gains scenarios 8-9 and the drag-reorder deferred item in both
languages; llm-replay README's zh side catches up with the { patches }
paragraph; pairings re-recorded.

Tianyi Cui f2972e846d fix(subprocess): keep awaited tree waits and the pending SIGKILL ref'd (Codex round 2)
An awaited waitForExit()/dispose() must hold the event loop open until the
tree really exits: with the liveness tick and the escalation timer unref'd,
a parent with no other live handles could exit claiming quiescence and
orphan the survivors it promised to reap. The escalation's pending SIGKILL
is a commitment; it self-bounds at graceMs. Module graph picks up the
subprocess-local -> timeout edge.

_Kerman 2985bc5267 test(goal-session): pin the unsettled-attempt yield with a real failure path
The drive-pass guard for an unsettled attempt was wrongly annotated
unreachable after admission joined the running interval. It is reachable:
a contained turn-close failure (pre-commit turn/end rejection) reaches
idle with the attempt's turn open and no terminal reason, and the idle
drive pass must yield to that attempt instead of misreading the absent
reason as settled. Replace the ignore with a test driving exactly that
path; the guard's comment now names the real producer.

_Kerman 050090e512 chore: regenerate catalogs and settle the goal driver's drive-pass guard
The admission-ordering fix changed the event taxonomy's observable order,
so the generated event/producer catalogs and the cordis API catalog are
regenerated. goal-session's mid-drive yield (an unsettled attempt seen by
an idle-triggered drive pass) became unreachable now that claimed prompt
admission joins the loop's running interval — a drive pass requires idle,
so it can no longer observe an unsettled reservation; the guard stays as
an annotated backstop.

Tianyi Cui 79e72eb736 fix(ui-primitives): prototype-safe alias lookup; pre-warm shiki off the render path
Responding to ds-review-bot round 2 on #662:

- LANG_ALIASES is a Map: an assistant-authored fence label like
  constructor or __proto__ now misses (plain render) instead of resolving
  an inherited object property and crashing shiki mid-conversation. Test
  sweeps the inherited-key labels.
- The singleton is pre-warmed in a deferred task at plugin boot (the
  ~120-175ms engine+grammar construction long task moves off the first
  finalized fence's render); the lazy path remains the correctness
  fallback, and unref keeps non-browser imports from pinning the loop.

_Kerman 2bc994900b fix(agent-loop): order admission publication and gate idle on committed turns
Three ordering fixes on the admission and settlement boundaries.

kick() installs the abort owner, marks the interval busy (running is
emitted before any listener can observe the claim), and installs the
pending done BEFORE publishing agent/inbox/dequeue: a dequeue listener
that cancels or disposes now finds live cancellation and quiescence
ownership instead of the previous activity's settled state, and claimed
prompt admission — including asynchronous prompt-submit hooks — sits
inside the running interval where cancel routing can reach it. The
admission-rejected path yields one microtask before continueOrIdle so
the idle transition cannot fire inside send()'s synchronous extent.

agent/idle now names only committed turns: a run that aborts or fails
before its turn/start commits exits without the notification, since
there is no durable turn/end for settlement consumers to act against.
The event's JSDoc states the narrowed contract.

Tianyi Cui c3c10820ba fix(tools): bound the shaped-append side channel; total error containment; recorded spill snapshot
Responding to ds-review-bot round 2 on #661:

- logWork is bounded: past maxParallelSubCalls pending shaped-append tasks
  the ordered commit lane holds (Promise.race drains one), so a slow spill
  backend backpressures the run instead of accumulating unbounded pending
  I/O and retained results. Tasks self-remove on settlement; run
  settlement still drains every task inside the open turn. New spill test
  drives three oversized reads against a hung backend at cap 1 and proves
  the third dispatch cannot start until a save drains.
- shapeDispatchLog's catch uses errorMessage() (total), so a thrown value
  with a throwing toString cannot escape the containment and lose the
  settle event.
- CodeDispatchLog.content documented as the RENDERED result projection
  (native tool/result vocabulary), not what the program received — the
  program gets the structured value; doc pair + type-equiv re-synced.
- New RECORDED tui-agent snapshot scenario code-mode-dispatch-spill: the
  real Loader-visible composition (worker runtime + spill-local + policy)
  drives an oversized bash sub-call end-to-end; replay proves the durable
  dispatch copy is bounded to preview + locator while the program value
  stays whole (the outer result carries just the line count).

kingwl 95315386b3 refactor(telemetry): remove the OTel backend's flush forwarding
Three review rounds each found a new silent-loss path in the same
wrapper state (dispose racing an in-flight flush, overlapping hints
displacing the retained promise, the provider's fixed 30s flush timeout
rejecting while the processor still drains). Every path exists only
because forwarding the seam's turn-boundary hint to forceFlush() made
this backend the process's second flusher against undocumented SDK
internals from the upstream experimental tree.

The backend now implements no flush(): the batch processor is the only
flusher, its scheduledDelayMillis (already deployment-tunable through
the processor passthrough) governs export cadence, and shutdown()'s
drain is complete by construction. The two race-pin tests collapse into
one dispose-during-in-flight-batch drain pin; the seam's optional
flush() contract now tells implementers they own the concurrent-flush/
shutdown interaction. Removal rationale and the reinstatement trigger
(a stated turn-boundary latency requirement scheduledDelayMillis cannot
meet — and then via the processor's own forceFlush(), never the
provider's timeout-wrapped one) are recorded in the revival Agent Note,
both languages.

_Kerman 5e8225505c refactor(agent-loop): state slot invariants directly instead of guarding them
The coverage pass left agent.ts with eight v8 ignore annotations — well
above this repo's density elsewhere. Six guarded conditions the
surrounding invariants already exclude; expressing the invariant directly
deletes the dead arm instead of excusing it: admission/run teardown
releases the abort slot unconditionally (no writer can replace the owner
mid-flight), the request-failed branch closes its always-open step
unconditionally, the recovery finally retires the window it installed,
and the driver finally drops its dead last-resort step close (every path
already closes the step). kick()'s dequeue states the non-empty-queue
invariant as a non-null assertion. The two remaining ignores are the
assertNever exhaustiveness guard and the retry()/run() slot-race guard,
both repo-wide conventions.

Tianyi Cui 79a28ad6d9 fix(subprocess): tree-scoped escalation and byte-exact tails (Codex round 1)
(A1) terminate()/dispose()/service teardown keyed on direct-child settlement
could leak a TERM-trapping descendant that outlived the leader (Codex
reproduced it with a disowned trap-SIGTERM helper). kill()/terminate() now
gate on tree liveness instead of outcome settlement; the SIGKILL escalation
timer survives settle (unref'd, re-probing the tree); dispose's tier
quiescence is whole-tree exit via a bounded waitForExit; the service's live
set releases handles only when their tree is gone, and its teardown awaits
tree exit. Three new suites pin the survivor scenarios end to end
(terminate, dispose, service teardown).

(A2) the escalation branch is now real tested behavior — its ignore is gone;
the one remaining signalTree guard ignore states why it is unreachable
through the handle verbs.

(A3) docs contradictions fixed: the impl README's stale POSIX-only bullet
now states the contained best-effort Windows tree story; the lsp-local
README no longer claims taskkill failures stay visible (containment + the
tree-liveness wait is the actual contract); the architecture tables (en+zh)
list all three consumer families.

(B1) OutputCollector keeps a byte-exact tail across uneven chunk boundaries
(trim the head chunk instead of dropping it whole) — the LSP diagnostic-tail
contract; pinned by a cross-chunk test.

(B2) the subagent-acp coverage ignore is narrowed to exactly the
never-settling success arm.

Tianyi Cui 3672cd25b4 feat(subprocess): migrate lsp-local, subagent-acp, and the env scrubs onto the seam
Review direction (tianyicui, PR #660): in a stacked PR, change all other
process-running places to use the new service.

- lsp-local: LspConnection spawns through ctx.subprocess (piped protocol
  streams + a no-spill collected stderr tail); its private process-tree
  helpers (POSIX group signalling, Windows taskkill, liveness polling) are
  deleted in favor of the seam's handle verbs, and its buildChildEnv now
  rides scrubbedParentEnv (LSP children also stop inheriting stale DSH_*).
  The plugin injects 'subprocess'; compositions/tests mount
  dsh-subprocess-local.
- subagent-acp: the ACP child spawns through the seam (piped ndjson streams,
  inherited stderr); spawn failure surfaces through done-rejection into the
  same startup race; disposal is handle.dispose with the plugin's configured
  graces. dsh-subagent-subprocess is DELETED — its dispose ladder and scrub
  are the seam's, and the isolated-config-dir helper had no consumer.
- mcp-client, pty-local, sdk-helper: adopt scrubbedParentEnv as the one
  scrub definition (their spawns stay put by ownership: the MCP SDK and
  node-pty own those calls; the SDK wizard runs outside any composition).
- Coverage: per-file 100% over every touched src file, with each v8 ignore
  carrying a platform or contract reason; new suites cover stdio
  dispositions, the dispose ladder tiers, injected-win32 tree semantics,
  waitForExit, settled-kill/terminate no-ops, and spawn-failure disposal.
- Docs: consumer-migration Agent Note (en; zh follows in this PR), seam note
  updated in place, subprocess.md rewritten for the reshaped vocabulary
  (type-equiv re-registered), READMEs and SERVICE_ROLES updated, taskkill
  added to knip ignoreBinaries.

Tianyi Cui f9cc62266c fix(tools): single ordered driver lane for the sub-dispatch scheduler; validate the cap
Responding to ds-review-bot round 2 on #658 (three critical findings, one
warning — all rooted in the pump/commit split racing ordered stages):

- ONE driver lane now owns every ordered stage: the start append, prepare
  (pre-execute/guards), and the head-of-line commit (post-execute, context
  deferral, settle append). start() is awaited before the next entry can
  start, so concurrent submissions can no longer run pre-execute pipelines
  concurrently; only the around-dispatch/body stage overlaps, matching the
  native loop's fillPool sequencing.
- An exclusive call's barrier now holds through its COMMIT: later starts
  wait for the exclusive pipeline (post-execute included) to finish, the
  native exclusive-group semantics.
- drainDispatches() awaits the driver run itself, so a commit already
  mid-flight when the program returns is drained before run_code closes
  the turn — the settle event and deferred contexts land inside it.
- maxParallelSubCalls is resolved and validated at construction (positive
  integer), so direct construction can no longer wedge the pool with 0.

New tests: overlapping-submission ordered-prepare, barrier-through-commit,
drain-mid-commit, cap rejection. 96 keyless snapshots replay unchanged;
Agent Note updated (both languages).

_Kerman 6a068ec28d test: restore 100% per-file coverage for agent-loop and the acp bridge
agent-loop: behavior tests for retry-while-busy, cancelled recovery
windows, no-facts stream failures, idle-listener preemption, rejected
driver promises under whenIdle, finish-chunk failures after step close,
presentationMeta persistence, pre-aborted and torn-down create/resume
signals, and configured-start failures over existing artifacts or after
teardown. The remaining guards that no public path can reach carry
justified v8 ignore annotations naming the invariant that starves them.

acp bridge: cover the retry-adoption path (a retry turn resolves the
prompt the failed turn deferred), the no-retry quiescence rejection, and
the admission-blocked cancelled settlement; the synchronous send-throw
catch is annotated as a future-proofing guard since the machine's send()
contains listener failures.

Tianyi Cui acf0d42ed8 fix(client-runtime): settled-only dispatch index carries null callTime; README matches
Responding to ds-review-bot round 2 on #653: the tool/code-dispatch event
is appended at settlement, so using its time as callTime fabricated a
zero-duration call for duration-aware consumers — it is now null (start
unknown) per the ToolResultNode contract, pinned in the session spec. The
README's codeDispatches section described the PR3 running→settled
lifecycle a stack ahead of this tree; it now documents the settled-only
index this PR ships (the running shape lands with the start event in
#658, which already merges cleanly over this).

Tianyi Cui 28b617dd73 test(ui-primitives): cover the fence pre-routing arms; drop the unreachable array probe
CI coverage flagged MarkdownText's pre route: the array-element probe
(raw[0]) and the mixed-content fallbacks were unreachable — the markdown
pipeline hands pre one code element whose children are one string (or
none, for an empty fence). Simplify to the string check, annotate the
isValidElement guard as representation-change armor, and pin both live
arms: the empty fence keeps the stock <pre>, a language-less fence renders
the plain CodeBlock arm.

Tianyi Cui 3b63bcbeec test: cover the dispatch-log seam's contained-failure and decline arms
CI's full-tree coverage flagged three untaken paths this PR introduced:
- shapeDispatchLog's catch (a throwing tools/code-dispatch-log listener
  must be contained — the settle event logs the unshaped content);
- the spill listener's flatten-decline arm (non-text sub-result content
  passes through unchanged);
- the generated scope-key extractor row for tools/code-dispatch-log
  (registered in the scope invariant matrix like the other tools events).

_Kerman fc8d339bf0 fix(acp): settle admission-blocked and retried prompts after the merge
The automation bridge inherited two master-era assumptions the message
machine no longer honors. A prompt blocked at pre-turn admission opens no
turn, so no turn/end could ever settle it — the bridge now watches
whenIdle() and reports a turnless slot as cancelled (the disposed-agent
guard moved to a registry identity check before send). A failed turn no
longer rejects at its turn/end either: agent.retry() closes the failed
turn and opens a successor on the same history, so the bridge holds the
terminal error and lets a retry-triggered turn/start adopt the prompt,
rejecting only at quiescence with no successor.

Also: refresh the empty-response-retry fixture for retry-as-turn logging,
adapt master-side tests to the unified send()/UserMessageData API and
registry-fact disposal, resync the doc pairs both sides touched, trim
architecture.md back under its word ceiling, and regenerate the event and
persistence catalogs.

Tianyi Cui 835156b038 fix(ui-trajectory): timing provenance on sub-span lanes; assembled snapshot for both views
Responding to ds-review-bot on #664:

- SubSpanLane gains a 'timing' discriminant (measured | running | unknown).
  A settle-only replay entry (callTime null, start outside the window) was
  previously indistinguishable from a measured 0 ms span; it now renders
  hollow with a 'duration unknown' hover title, and durationMs stays null
  for anything unmeasured. Pairs with the client-runtime fix that stopped
  fabricating callTime = settle time (826c3696a on the live-parallel PR).

- The built-client Code Mode fixture snapshot now switches to the
  Trajectory and Waterfall tabs and pins the assembled rendering: three
  Sub cells with real +0.8s durations and three measured lanes with their
  hover titles — product-visible coverage through the real bundle graph,
  not just package-level jsdom fixtures.

Agent Note (both languages) updated for the timing contract; pairing
re-recorded.

Tianyi Cui f1b7d52a77 fix review findings: hostile code accessor + swallowed teardown
Both ds-review-bot findings were real:

- markLlmAdapterFailure's carried-facts cross-check read error.code
  directly; a foreign Error with a valid own failure payload but a
  throwing code accessor would replace the original adapter error with
  the accessor exception, breaking the error-identity guarantee. The
  read now goes through foreignErrorCode(), which contains the trap and
  falls back to the normalized snapshot (test: hostile code accessor
  beside a valid failure payload -> original identity kept, UNKNOWN
  facts).
- live-interactions' afterEach caught scaffold.close() into undefined,
  silently disabling ReplayHandle.assertConsumed() — the fixture-drift
  tripwire — and hiding cleanup defects. Teardown now runs every step,
  collects failures, and rethrows (AggregateError when several).

Tianyi Cui 12a7e38417 feat(subprocess): reshape the seam Node-ward for multi-consumer use
Review direction (tianyicui, PR #660): make the interface closer to Node's
API so the other process-running places can adopt it. The spec gains
per-stream stdio dispositions — 'pipe' (raw Readable/Writable for protocol
streams), 'inherit' (diagnostics to the parent), and collect mode ({maxBytes,
spill?} — the old bounded tail-keep shape, now with spill optional for
diagnostic tails). SubprocessOutcome carries exit facts only; collected
output stays readable through handle.collected after settlement (spill fds
are sealed at the settle boundary). The handle grows Node-style kill(signal)
(single signal, tree-scoped, no-op after settlement), terminate() (the
SIGTERM→grace→SIGKILL escalation, also driven by the spec signal),
waitForExit() (tree liveness, not just the direct child), and dispose()
(the cooperative stdin-EOF→SIGTERM→SIGKILL ladder from subagent-subprocess,
graces caller-supplied). Tree semantics are platform-correct: POSIX detached
groups with direct-child fallback; Windows taskkill /T with an injectable
runner. scrubbedParentEnv/SENSITIVE_ENV_PATTERN move to the seam as the one
shared scrub definition.

bash-local maps its config onto collect modes and batch stdin and reads
results through the collected readers; its kill() maps to terminate() so
task_kill keeps escalation semantics.
